YIF Initiative ยท Direct Care

Emergency Support
Outreach Clinic

A direct care initiative providing immediate, life-saving support to marginalized young people, moving from crisis response to physical delivery of care, resources, and dignity.

Where This Began

From Hotline to Clinic

The Emergency Support Outreach Clinic grew directly out of YIF's Emergency Support Hotline: a rapid response service built to connect individuals in crisis to critical resources: mental health support, legal aid, emergency shelter, and safety assistance.

The hotline served as a trusted, confidential bridge between people in danger and the services that could help them survive and recover. But a bridge is not enough. The Outreach Clinic is the next step - moving from referral to presence, from response to care.

By establishing regular outreach cycles in underserved communities, we deliver immediate relief while building something longer-lasting: a culture of resilience, belonging, and hope.

What the Clinic Provides

Direct Support, Delivered

The clinic is designed to meet people where they are: physically, emotionally, and practically. Every service offered addresses a real, immediate need.

Mental Health Support

Confidential, affirming mental health care for those experiencing trauma, crisis, or the psychological weight of violence and exclusion.

Legal Aid

Access to legal support and guidance for individuals who have experienced entrapment, abuse, or rights violations tied to their identities.

Emergency Shelter

Safe housing referrals and emergency shelter support for those facing homelessness or displacement as a result of violence or exclusion.

Food & Basic Necessities

Food supplies and essential goods for those facing hunger and material deprivation, ensuring access to basic sustenance between outreach cycles.

Safety Assistance

Immediate safety planning and protective support for individuals at risk of physical harm, targeting, or violence related to their identity.

Community Connection

Beyond material support, the clinic offers belonging: a space where individuals are seen, respected, and connected to a wider community that affirms their dignity.

How It Works

The Outreach Cycle

The clinic operates through structured, recurring outreach cycles; not single visits. Each cycle is designed to build on the last, creating continuity of care for individuals and communities.

01

Reach

Outreach teams go directly into underserved communities, meeting people where they live, not waiting for them to come to services that may feel inaccessible or unsafe.

02

Respond

Immediate needs are assessed and addressed on the spot, whether that is food, safety, mental health support, legal guidance, or a referral to specialist services.

03

Sustain

Between clinic visits, beneficiaries receive resources to carry them through. The goal is not one moment of relief but a sustained safety net that holds people up over time.

Who the Clinic Serves

Reaching Those Left Behind

The clinic is designed specifically for those who face compounded layers of exclusion. Not those who have been inconvenienced by systems, but those who have been abandoned by them.

Our focus is on grassroots communities that are most at risk and least served: the individuals who cannot wait for services to find their way to them.

Young people who have experienced physical abuse, entrapment, or identity-based violence
Those facing unemployment, homelessness, and food insecurity due to socio-economic exclusion
Sex workers, people living with HIV, people with disabilities, and people who use drugs
Individuals in disregarded and disadvantaged grassroots communities with limited access to formal services
